Discuss the Relationship Between Great Britain

The years 1750-1776 were very volatile years regarding the relationship between Britain and the North American colonies. A large scale war, taxes, and revolts all played a part in the hostilities between the two nations. Economics, however, may have been the spark that started the fire between the two nations, and might perhaps be the most significant cause of the future conflicts between the North American colonies and Great Britain. The French and Indian war was fought from 1754-1761 primarily between the French with Indian allies and Britain for control of North America.This war was one of the first wars that would be fought on a global scale requiring more resources to aid in the war effort. Britain poured very large sums of money into the war in order to protect the colonists. By the end of the war, after Britain’s victory and the signing of the treaty of Paris of 1763, Britain had expended much of its resources. Americans, after hearing the news of Britain’s victo ry in the war, were readily making plans to venture westward into newly acquired lands. Britain, fearing that Indians would attack the colonies, quickly passed the proclamation of 1763 prohibiting settlement west of the Appalachians.Doing this was one effort to prevent future conflicts that would in turn require Britain to utilize more of its resources. Britain had dug an economic hole so deep that it would require vast amounts of dirt to refill it. Where was the money to come from? The Americans, having barely supported the war effort, would become the mule that was to carry the burden. The war was in fact fought to protect the colonies; thereby the colonists should have to pay for the money that was used for their own defense. Britain embraced this idea and started passing tax acts and enforced the navigation acts.Of the acts that were passed to raise tax revenue the first was the Sugar Act. This act placed a tax on all sugar that was imported into the colonies. Another act that w as passed was the stamp act which placed taxes on playing cards, marriage licenses and other printed goods. The colonists showed fierce resentment to this as they eventually formed the stamp act congress. As seen in Document B sections III and IV the colonists believed that they did not need to pay these taxes as they were not even being implemented by their own government.Other forms of resentment would arise as groups known as the sons of liberty and daughters of liberty would be formed. In document C it is shown that the sons of liberty were a group of common townsfolk workers. These men and women would cry for liberty, no stamps, and property as they believed that they should not have to pay for the taxes. They also enforced nonimportation agreements to make other fellow colonists support their cause to rebel against the mother country. The navigation acts also posed as a serious threat to the colonists as they forced trade specifically between them and England exempting other c ountries from the trade.This was a major economic problem for the colonists as they were not able to sell goods to other countries as they could possibly obtain larger profits as they could sell more goods and at higher prices. In document D Benjamin Franklin talks about how the colonists are restricted to trade with England except for the smuggling that occurs between Spanish and colonial trade. He feels that some action should be taken that would eventually better the colonies economically. Overall, economics were the roots of the weeds that had been planted by Britain sparking a major conflict that cost them an entire nation.

Power in ‘Ozymandias’ (page 14) and in one other poem from Character and voice: Compare Essay

Bothe Ozymandias and The River God have a strong theme of power throughout the poems. Power in The River God is first present when the river says â€Å"But I can drown those fouls† The use of the word â€Å"can† brings out the power of the river. He has the choice over people’s lives and the river is not afraid to let us know this. The whole sentence sounds like The River God is trying to add fear to whomever he is addressing, showing them he is in charge and they should know this. Ozymandias also attempts to bring out fear through his power. â€Å"My name is Ozymandias, king of kings: Look on my works, ye Mighty, and despair!† He is confident that he is more powerful than the mightiest and wants everyone to know this. The poet uses the word â€Å"despair†, in other words Ozymandias is saying look and be afraid. The word shows the worry and fear of those who lived in the times of Ozymandias. Both Ozymandias and The River God believe they are more powerful than any other, they are both arrogant and do not see anyone as competition to their reputation. The River God says â€Å"O I may be an old foul river but I have plenty of go†. He is saying that even though he is worn down and old he is still as strong and mighty as he was when he was a young river. The use of the word â€Å"plenty† brings out his arrogance, he is not just saying he is powerful but he is saying he will carry on with this strength for years to come. However in Ozymandias there is proof of his down fall, his loss of power. â€Å"Nothing beside remains† and â€Å"a shattered visage lies† are both evidence that even though he had all this power once he died it all left. The poet could have done this to show the reader that nothing lasts, everything has to end. The use of the word shattered is strong as it is almost like somebody defeated Ozymandias and that somebody was time. Als o it is quite ironic that on the stone it was written â€Å"Look on my works† as none of them remained and therefore couldn’t of been as important as he made out. Ozymandias is written to sound powerful when read out loud, iambic pentameter is used which gives the poem a regular sound.

Barry who is John’s supervisor taunts John and encourages other employees to treatment him badly because of his condition. What’s more, we can find an explanation in the pages of God’s word as to why the school system is going downhill when it comes to God and morals. The passage at 2 Timothy 3:1-5 gives us a good explanation as to why such events are transpiring. This passage talks about the last days and what is to occur during those times. We are told that children will be disrespectful to those in charge, having a complete lack of regard to authority. Violence will increase greatly, as people will commit violent crimes without a conscience. There will be a serious lacking of self control, and the number of selfish individuals will increase to an alarming rate. Should a person make a mistake, there will be no forgiveness if he wants to smooth things over because people during the last days will be very unforgiving. And if this isn't bad enough, we are told about how people will have a prideful spirit, and will deeply persecute those who love serving God and doing what is right. People will only be concerned about the things that pleasure them rather than devoting time to serving God (the Holy Bible, New King James Version.) Everything that was mention in this Bible passage is characteristic of what is currently occurring in our schools at present. Years ago, children actually respected the teachers. Now, it is not common to hear that teachers stand around helplessly while the children do their dirty business because they are afraid of what the children might do for correcting the behavior. Children talk back to the teachers, and some have even inflicted harsh physical violence. The number of violent incidences that occur in our schools today is ever increasing. Just about every time one switches on the TV to watch the news, there is some sort of report on school violence. There are numerous cases concerning children who bully others, and nothing is done about it because the teachers are afraid of the bullies. Others take things to the extremes by calling bomb threats or bringing weapons to school to injure and kill teachers and fellow classmates. Such violent behavior is not characteristic of a Christian who is reverent in his service to God, and it shows us that things are really getting out of control as described in the above Bible passage. When we look at the pages of God's word, He makes it very clear how he feels about violence. At Galatians 5:19, 20, there is a list of practices that are not acceptable for a servant of God to engage in. Acts of violence are listed among them. In these verses, we are told what will happen to those who engage in such acts. These persons will not inherit God's kingdom (the Holy Bible, New King James Version). Also Psalm 11:5 states that God hates violence (the holy Bible, New King James Version). Nothing can be clearer about how God feels than this verse, which directly states His feelings about the subject. The mode of knowing is obvious in the use of the word apparently. Implicitly, the speaker is not conveying direct knowledge of a fact but rather an inference is drawn from information. The use of the word apparently is demonstrative of the speaker’s unwillingness to commit to a definitive conclusion but rather a willingness to offer an interpretation or perception of the information received or observed (Brinton, 1996). The speaker limits the information to an inferential form and is thus manifesting a degree of uncertainty by using the word apparently and failing to assert unequivocally that the information observed or received can be conclusively stated. Faller (2002) describes what is known as the reportative marker. The reportative marker is relevant to the word apparently because it offers a contradictory stance on the part of the speaker.

Introduction Childhood obesity is a major focus of the society and the public health officials. And childhood obesity trends have increased dramatically over the past several years (Staniford, Breckon, Copeland, 2012, p. 545). Overweight is measured by body mass index which is called BMI. According to the survey from the 2003 to 2004 National Health and Nutrition Examination Survey (NHANES), overweight increased from 7. 2% to 13.9% among children during 2 to 5 years old and from 11% to 19% among children who are 6 to 11 years old. And among 12- to 19-year-olds, the statistics of overweight increased from 11% to 17.1% from 2003 to 2004 (Faith, Kerns, Diewald, 2009, p.281). In the past several years, the statistics of childhood obesity has a dramatic global increase. Therefore, childhood obesity is becoming a more and more serious problem. According to Ebbeling, Pawlak and Luwing (2002), â€Å"[b]odyweight is regulated by numerous physiological mechanisms that maintain balance between energy intake and energy expenditure† (p.474). The factors that cause childhood obesity include four aspects: (1) Genetic, perinatal, and early-life factors, such as â€Å"five genetic mutations that cause human obesity have been identified, all presenting in childhood, and the BMI normally decreases until 5-6 years, then increases through adolescence†; (2) Physical activity, children who are lack of exercises are easy to be obese, for example, television viewing is thought to promote weight gain because of keeping sitting; (3) Diet, for instance, fast food almost comprises all of the potentially adverse dietary factors, â€Å"including saturated and trans fat, high glycaemic index, high energy density, and, increasingly, large portion size†; and (4) Family factors, the risk of obesity is effected by the parent-children inactions and the home environment, for example, many families like to eat in restaurants which tends to serve larger portions of energy dense food (Ebbeling et al., 2002, pp.474-476). Nowadays, chil dhood obesity is becoming more and more serious. It has been identified as a harmful epidemic all over the world, especially in the U.S., Brazail and China (CIA World Factbook, 2009, para.7). As childhood obesity causes many negative effects both on children themselves and on the healthcare system, it is important to pay attention to the possible solutions for childhood obesity. 2. The Negative Effects of Childhood Obesity Childhood obesity has negative effects on childhood life which include intelligent and health complications. In other words, obese children would meet many problems during their usual study and life. People need to concern the health complications which are associated with childhood obesity, including â€Å"Elevated blood pressure, hyperinsulinemia and glucose intolerance, respiratory abnormalities, poor body image, and increased adulthood mortality in females† (Faith et al., 2009, p.281). This reveals that childhood obesity has a significant negative influence on the maintenance of a healthy body, even in the future of childhood life. In addition, childhood intelligent quotient (IQ) is associated with childhood obesity. According to the research, â€Å"the FIQ [full intelligent quotient] and PIQ [performances intelligent quoient]of obesity in school-age children were lower than normal weight children† (Yu, Han, Cao, Guo, 2009, p.665). In other words, obesity would mak e children face problems such as depression, social isolation, low self-esteem and poor academic performance. Therefore, overweight children need to face more problems on health and intelligence than normal weight children. In addition, childhood overweight and obesity cause not only childhood life problems, but also the financial burden to the public health system. Obese children cost much higher publicly funded medical and pharmaceutical costs than normal weight children. According to Au’s research, â€Å"the financial burden of overweight and obesity occurs even during the first 5 years of primary school†, and â€Å"the prevention of overweight in children as young as 4-5 years could have significant economic (in addition to health) implications† (Au, 2012, p.670). This clearly shows that childhood obesity is the financial burden to public medical and pharmaceutical costs. Therefore, from an economic perspective, it is necessary to control the trends of childhood obesity. 3. The Treatments of Childhood Obesity From the negative effects that are talked about, I discuss that it is necessary to find effective solutions to solve the problems of childhood obesity. There are four solutions: (1) Diet modification, (2) Physical activity, (3) Psychotherapies for managing obesity, and (4) Parent participation. 3.1 Diet Modification Diet Modification is the most useful and effective pattern for obese children losing weight. What obese children firstly need to do is to change the dietary behaviors and keep healthy dietary strategies. As Faith et al. points out, â€Å"behavior modification strategies, such as behavioral contracting, stimulus control, and/or a specific dietary plan, are required to help children lose weight (Faith et al., 2009, p.291). This reveals that it is powerful and useful to make a healthy dietary component such as moderate caloric restriction and low-fat diets for overweight children. In addition, making good food choices is also a good way to control obese children’s weight. It suggests that exercise therapy is essential to the maintenance for losing weight. According to Epstein (1995), there are some â€Å"short-term effects of physical activity interventions on both children’s weight status as well as their cardiorespiratory fitness and other cardiovascular health benefits† (as qtd. in Faith et al., 2009, p.293). This clearly shows that physical activity is an effective way for obese children both on losing weight and keeping their important organs healthy. Furthermore, the therapy of physical activity for obese children is applied to physical education (PE) programs in schools. To sum up, physical activity is an essential treatment for overweight children to lose weight and keep healthy. 3.3 Psychotherapies for managing obesity Furthermore, there is another one which is called psychotherapies for managing obesity which are accepted by many overweight children’s parents. Psychotherapies for managing obesity can effectively change people’s behaviors about their unhealthy lifestyles. Dialectical behavioral therapy (DBT) is the most useful therapy in psychotherapies for managing obesity, and it â€Å"does not focus directly on binge episodes or on reducing bingeing, but rather on the dysregulated affect that can trigger binge eating† (Taylor, Stonehocker, Steele, Sharma, 2012, p.15). In other words, DBT can successfully stop obese children from going on a binge, and help children to control themselves about their diet behaviors. In addition, there is another powerful treatment in psychotherapies for managing obesity. Taylor et al. also introduce motivational interviewing (MI) in their research, â€Å"MI is a strategy designed to enhance patients’ motivation for change and adherence to treatment and is fundamentally different from educational approaches† and â€Å"this type of approach may be particularly well suited to weight loss† (Taylor et al., 2012, p.15). In other words, in weight management, this behavior can make obese children follow plans for losing weight effectively, and make them insist on the strict diet component which helps them to lose weight. Therefore, psychotherapies for managing obesity can be successfully applied to solve the problems about childhood obesity, although this kind of treatments is controversial among parents. 3.4 Parent Participation Finally, the benefit of parental participation in childhood weight-loss processes has drawn attention from the whole society. In other words, Parental participation is a useful and effective pattern to solve the issue about childhood obesity. According to Golan et al. (1998), the research shows that â€Å"parental participation did not improve effect found for treating children alone. Although other data suggests that treating parent alone may be more effective for inducing child weight loss.† (as qtd. in Faith et al., 2009, p.295). This demonstrates that it would be better if parents take part in their children weight-loss programs, and they can make weight-loss programs effectively. In addition, parental participation can be better for obese children’s healthful eating. Faith et al. suggest parents that â€Å"[n]ever use food as a reward; establish daily family meal and snack times, offer only healthy food options; he a role model for children; and parents or caregivers should determine what food is offered and when, and the child should decide whether to eat† (Faith et al., 2009, p.295). This clearly shows that parents should focus on increasing praise for children’s healthy diet behaviors and activity choices. They also need to better plan the whole family eating and physical activity programs, and parental modeling. As a result, parental participation in childhood weight-loss programs has a positive effect for obese children losing weight. 4. Conclusion Childhood obesity trends have increased dramatically during the past several years, it is important for people to pay attention to the treatments of childhood obesity. There are four main reasons for childhood obesity: Genetic, physical activity, diet and family factors. These factors make children face problems about childhood obesity, and have negative effects on both childhood life and society. As a result, there are four useful and powerful solutions for children weight-loss programs: (1) Diet modification, such as change unhealthy diet behaviors; (2) Physical activity, which suggests children to do more excises; (3) Psychotherapies for managing obesity, which is a way to use the theory of psychology to cure obese children; (4) Parent Participation, which advising involves patents to take part in children weight-loss programs. Through the whole research, we see that childhood obesity is becoming more and more serious in contemporary society. Routine Activities Theory (RAT) Analysis Activity space consists of places people associate with in their daily lives. For instance, your home, work, school, places for entertainment and shopping areas are considered activity space. My activity space is a tri-county area which means that it is a large area that consists of smaller towns instead of a huge metro area. Since I commute to work my activity space consists of major travel nodes and pathways as described by Brantingham and Brantingham. These travel nodes and pathways would be interstate 80 (I-80) that runs West through my town and state route 422 which runs South of I-80 and directly into the city of Youngstown. During my routine I tend to use both I-80 and 422 because they are quicker routes which allow me to avoid passing through a bunch of smaller towns and is a straight shot to the city. Within my activity space the highest potential for the development of a crime hot spot would be toward the eastern portion of route 422 going into the city of Youngstown becaus e it is an area between the city and the suburbs that does not have a strong police presence. This particular area is residential and in close proximity to the high-risk areas (i.e. project/public housing development) and has a wide-range of crime generators and attractors such as businesses, homes, bars, gas stations, convenience stores, and heavy pedestrian traffic due to a nearby bus station. The location of travel routes in conjunction with environmental factors such as impoverished surrounding neighborhoods and the close proximity to major travel routes can be seen as an opportunity for offenders to commit crimes within this area. Brantingham and Brantingham (1999) state that crime generators are particular areas that attract people because of the large number of people that pass through them. this activity space, as described by Brantingham and Brantingham is between the major nodes of transportation and is close to districts in the city that could be seen as an opportunity, to a potential offender who has observed routine activities conducted by pedestrians, members of the neighborhood, and business owners in the area. The routine activities theory draws from Amos Hawleys theory of human ecology. Hawleys theory establishes three key aspects of collective human activities rhythm, tempo and timing which are factors in a life-course as well as the criminal lifestyle. Lawrence Cohen and Marcus Felson (1979) expanded on the principles of human ecology and introduced routine activities theory as an ecological perspective on criminal behavior. The routine activity theory states that there are three components in order for a crime to occur. The first component is a motivated offender or individual that is not only willing but seeking to commit offenses. The second component would be the presence of suitable targets. Suitable targets could be property or individuals that are seen as available or vulnerable. Lastly, the absence of a capable guardian which would be anything that could deter a potential offender (i.e. police patrolling, neighbor outside, alarm system). Cohen and Felson (1979) emphasize that these three components are conducive to a crime event and especially prevalent in the absence of capable guardians. An example of this would be a potential offender(s) attempting to burglarize a home but changes their mind after seeing a police squad car drive by. This type of guardianship applies to certain areas within the city especially those that are heavily patrolled by police or security (i.e. parking decks and high-risk areas). Furthermore, the capable guardian, whether it be an individual (police officer/witnesses/neighbors) or a device (home/store alarm) will reduce the suitability of a target, which in turn will decrease the likelihood of criminal events. Moreover, Cohen and Felson (1979) imply that a successful crime event doesnt require an offender who is motivated to engage in crimes, or act on their motivation for the criminal event, instead a motivated offender should be capable of carrying out their desires. According to the routine activities theory, crime opportunity emerges in the absence of a capable guardian as well as certain environmental factors. For instance, the offender also goes through a routine of sequential activites just like the law abiding citizen does. The combination of what is considered a crime template (the offenders routine intertwined with their prospective targets) and the decisions made by the offender can determine crime patterns. A crime is committed when a triggering event occurs. This triggering event is usually in place when a potential target or victim fits within the offenders crime template (or routine). Possible targets and victims will usually encounter the offender in some manner within their active location, resulting in sharing the activity space or the awareness space of the offender. The possible targets and victims end up being actual targets or victims once the offenders willingness to break the law is set off. This occurs when the nodes and pathways between these nodes are aligned with the offenders potential targets (at that place and time). When these activities are repeated on a daily basis they provide a rubric or template for the criminal to follow. During a criminals daily activity they make decisions that rarely vary from their routine and as a result, breaking the law would be no different from their normal activity and awareness space. Lifestyles or ones routine activities create criminal opportunity by increasing the frequency and intensity of contacts between potential offenders and suitable targets. When a motivated offender is ready and willing, they will seize the opportunity to engage in criminal activity if it is beneficial to them. A potential offenders suitable target is an individual or object that is attractive or vulnerable, such as, a piece of valuable property or someone who is wealthy might serve as a lucrative suitable target (i.e. ransom or extortion). In other words, factors that make a target vulnerable or attractive are crime specific and situational and may dictate a motivated offender(s) likelihood of criminal activity (i.e. an unattended running car a convenience store). I believe that the most likely hot spot in my activity space and based off of the course material, would be state route 422 east going into the city. I state this because this area is known for drug activity and has access to the highway and is within close proximity to a wide-range of businesses, churches, schools, and residential areas. Collaborative Working Reflective Essay Throughout this whole assignment I am going to critically appraise others and my own practice as a collaborative worker via personal reflections and experiences of collaborative working, through experience in professional practice. I aim to link service user improvement and collaboration defining the importance of them both. Furthermore, explaining the various leadership models clarifying why they are important and needed throughout a health care team. I will plan to explain and critically evaluate an experience with the intention to promote positive outcomes for the service environment. Additionally then identifying a service improvement plan, in this case designing a 15minute time management nutritional chart for patients with dementia. Service improvement The BW Quality Safety (2007) defines service improvement, stating it is a combined and constant effort from everyone, including healthcare professionals, patients and their families, researchers, payers, etc. The changes need to lead to better patient outcomes, better quality care and better professional development (see appendix 2). The aim of all health care systems strive to provide safe and good quality health care, improve patient experiences, tackle effectiveness and update practice in the light of evidence from research (RCN 2015). Critical analysis of own performance from the Interprofessional capability framework (2009) section OC3/L2, I identified myself as level 2 (see appendix 1). During my district nursing placement, collaborative working is a key when being a nurse in the community. During my placement I interacted with various health professionals across various organisations. I ensured I was knowledgeable about the information I was passing over and I always opted in to interacting with the other professionals to build my confidence. Collaborative working The King Fund (2014) recently released a new policy document about time for change bringing ideas together from all sectors to help change the health care and improve collaborative working. The Royal College of Nursing (2004) states collaboration is diverse, ranging from intra-disciplinary teams on an individual setting to multi-agency working practices. Collaboration covers the process of researching, assessing, planning, implementing and evaluation (Thomas 2014). Critical analysis of my own performance from the Interprofessional Capability Framework (2009) section R2/L2, I identified myself as level 2 (see appendix 1). In multi-disciplinary meetings (MDT), I was co-operative, keen and knowledgeable about the patients. I was eager and asked questions throughout the MDT. I asked question when not understanding and I felt as though the health care professionals valued me as a student because I showed an interested instead of doubting my own knowledge. Weaknesses showed as I felt more nervous to question a doctor if I didnt feel the statement was correct. Nevertheless, opportunities to share and discuss actions with area for improvements are valued within the health care system (RCN 1995). Additionally, critically analysing another Interprofessional Capability (2009) section CAEP1/L2. I identified myself as level 2 (see appendix 1). Through collaborative working I was able to achieve this capability by engaging myself in discussions about cultura l beliefs and awareness, during MDT meetings and general discussion between different sectors, therefore enabling to gain knowledge about the issues within communities of practice. Service user and collaboration The Journal of Nursing Management (2010) cited by Francis (2010, p400) dedication, compassion and effective teamwork contribute to the welfare of patients and should be valued. Both nursing and medical staff are entitled to effective collaboration, one of the core values of Interprofessional working should be about respecting the individuals within the team (Barnes 2012). Collaborative practice between disciplines, patients and family result in the highest quality of care and strengthens health care systems, proposing that Interprofessional education is the way forward to producing a collaborative-practice ready workforce (Goodman 2010). Reflecting on my first placement, collaborative practice was shown poorly within the team and there was little discussion made throughout the team. This made it difficult for crucial information to be passed on effectively. Critically analysing my own performance from the Interprofessional Capability Framework (2009) section CW/L2, I identified myself as level 2 (see appendix 1). Effective communication is one of the primary barriers when working to ensure safe, consistent and excellent patient care (Baird 2012). An area of weakness when I communicate with patients is posture, from self-analysis I have noticed that on some occasions I become awkward and am unsure where to stand or how to sit in front of a patient. Hopefully, through self-realisation I will be able to improve in this area on my future placement. Leadership The NHS health care system is subject to a pressure of change, throughout these changes the health care industrial requires nursing leaders with special attributes, therefore identifying leaders who are able to guide the profession into a positive future (Sofarelli 1998). The frameworks that will be critically analysed are The NHS Leadership framework (2011) and NHS Change Model (2013). The NHS Leadership framework (2011) to bring together leadership principles and best practice guidance. The framework delivers a reliable approach to leadership development for staff in health and care throughout the NHS. The NHS Leadership framework is made up of nine leadership styles (see appendix 3). The leadership behaviours are shown on a four-part scale which range from essential† through proficient and strong to exemplary. The NHS Change Model (2013) has a similar aspect about leadership with slight differences as it has been released more recently (see appendix 4). The leadership framework also encourages staff members at all levels across the NHS to become a leader and the main aim for this framework is to encourage everyone working in the NHS to become a leader of change, pushing for everyone’s opinions to gather a general scope of the main issues in the healthcare. So how do leaders inspire staff to participate? Staff members need to be able to be independent, ensuring they can widen their choice of skills (West Dawson 2012). This will allow greater job satisfaction. Leadership is important when influencing a group of individuals to achieve a specific and obtainable goal. The style of the leader is essential when influencing change and aiming to achieve a high quality of care. Within leadership there are various types of leadership styles which, depending on your personality, determine which style you will obtain. Collective leadership is known as the most popular leadership style used within NHS healthcare. This style is based upon building relationships with the other health service users, the individual is strong and has passion to support and grow the team (Jackson 2007). This type of style influences and motivates other members, facilitating the development of robust, vibrant and reproductive research cultures (Russell Stone 2004).The decisions are made within the whole team based on the organisations values and ideals. Additionally, authoritarian leadership is where all the decisions are made without consenting any of the other staff membe rs, negative reinforcement and punishment is often used to enforce rules. This type of style is used when the individual feels power and generally withdraws from the team. The positive aspect about this style is that in an emergency situation little discussion is made and this then enables tasks to be completed promptly. I felt that during first placement authoritarian leadership style was used mainly. This was due to a lack of staff and high demand from the patients. This style seers to be the best for this kind of situation but it also entails negative points. Critically analysing my own performance throughout placement, I personally feel that I am heading towards becoming a transformational leader, which is very similar to the collective leader. During my first third year placement, I had the chance to lead a small group of team members that were caring for the patients I was in charge of. I needed to make sure I had charisma and confidence, ensuring I motivated the other staff members and allowing me to build relationships with the team. At first I felt embarrassed and unconfident because of my experience compared to others, although after getting to know the team and showing commitment and knowledge, it allowed taking charge easier because I had more respect from the team. SECOND SECTION During placement periods as student nurses, we all experience different experiences and various routines dependant on the ward allocated to us. Throughout this section of the assignment, I am going to discuss a placement ward in which I felt there should be an area of change. The reflective model I have chosen to use is Bortons model (Barton 1970). Bortons model simply puts three simple questions to be asked of the experience to be reflected on; What?, So what?, Now what? The model will be incorporated into the reflection to facilitate critical thoughts, relating theory to practice. In my first year of becoming a nursing student, I was placed on a care of the elderly ward for dementia specialising in Parkinsons, with around roughly 26 medical beds. This ward was very fast paced and constantly hectic. Throughout the placement, I noticed the patients suffering from severe dementia had various nutritional needs. Weight loss is common in individuals suffering from dementia, caused by poor appetite. This could be due to a variety of problems including communication, depression and pain (Alzheimers society, 2013). I noticed that occasionally some patients would not have eaten throughout the whole day or even barely drank fluids due to refusing at meal times; this therefore becomes the patient’s routine because food isnt incorporated into their daily activity. The main issue with this ward was time management due to the high demand of patients and care needed. This sometimes showed to have a damaging effect on various patients that needed more care and time. PDS A cycle plan is to design a time chart which specifies that a minimum of 15 minutes one-to-one time, needs to be spent with a particular patient. This will then hopefully enable the patient to become familiar with you as the care giver during their meal time. It occurred to me when on a dementia ward that the patients often go by familiarity despite their memory. For example, some patients would only consume diet and fluids when their relative was around despite not knowing who they are. Therefore, hopefully with my change of plan being put in place, if a member of staff is allocated specific patients for the day and every meal time the care-giver spends 15 minutes with the patient during the period, the likelihood of the patient consuming even a small amount of food is higher than when the patient was not receiving enough quality time. Additionally to help implement the service improvement, structures known as process mapping and the PDSA cycle (plan, so, study, act) are used. Process mapping enables health professionals to capture the certainty of the patient experiences, following their whole journey to help identify the main problem areas for change. An example of a process map performed is shown in (appendix 5), designing a process map helps to identify the specific problem, which provides clear evidence that a service improvement plan is needed. In this case, a process map was not needed for this service improvement plan. On the other hand the PDSA cycle is used to provisionally trail a change in practice, allowing the team members and patients to assess the impact of the change before implementing it into practice. Plan To firstly initiate my plan of change I introduced it into the multi-disciplinary team meeting. This allowed me to help steer and co-ordinate the intervention as well as review my process with the team. Therefore bringing all of the health care professionals together can then be given a stake in the outcome and we can all work to achieve the goal. Clarke (2008) believes that teams without nurses are guaranteed to fail. Additionally tears led by nurses and therapists, however successful, often lack control; therefore doctors must also be a part of the team. This change of plan has been designed purely through observation during my first year placement. Speaking to various service users and family members I gathered together a concern for the patients nutritional needs. As well as noticing a strain on the staff I thought assembling a change of plan will relieve the team and prevent stress, hopefully bringing collaborative practice together. Additionally when implementing a plan of change there will be controversy. Lewin (1951) designed a force field analysis, a strategic tool used to understand what is needed for change in both corporate and personal environments. For example Kurt Lewin (1951) states directly An issue is health in balance by the interaction of two opposing sets of forces those seeking to promote change, known as the driving forces and those attempting to maintain the status quo (restraining forces). Throughout change there will always be individuals willing to contribute to make a change happen, nevertheless there will be restraining forces that resist. To help with the leadership section for my plan of change, the approach I will use is the transformational style. This will allow me to bring everyone together creating a discussion on everyone’s thoughts and feelings about the plan. It is crucial that the idea set out is agreed by the majority of the individuals, because the plan of change will cost a small amount from the NHS budget, therefore the change needs to be beneficial to the NHS. The Institute for Innovation and Improvement (2013) states currently in the NHS we are facing an unpredictable challenge to improve quality and reduce the cost. Collecting the correct data both quantitative and qualitative at frequent intervals over extended periods allows the health professionals to make an uniformed decision about whether the change is moving the NHS in the correct direction. To enable my plan of change to happen/work I am going to need to ensure I have the involvement of various team members. Therefore allowing the patie nts to get the specified 15minutes one-to-one time, obviously nurses and health care assistants are going to be my main priority. Do To test whether this change of plan is a good idea I am going to perform a pilot study. A pilot study is a methodological introduction, the aim is to develop, adapt and check the possibility of the methods functioning for my service improvement plan (Foster 2013). To test this idea firstly, I discussed the service improvement with members of staff from other wards and family members to gain a general scope of ideas about plan. I performed this because the ward used for my service improvement plan, staff did not work collaboratively therefore I didnt feel as though I would gain a positive outcome. Nevertheless I decided to use questionnaires with the whole team on the ward. This allowed me to collect the positives and negatives together and analyse whether I have achieved the service improvement. Additionally collecting the information will allow me to predict how long the process will take due to the amount of staff members that are for my service improvement. Main source of data has come from surveys and questionnaires using a qualitative research approach. Qualitative research is performed in a realistic setting, generally used from research that is collected through interviews and observation (Cleary 2014). Reflecting on this I am able to look back at the data collected and weigh out the pros and cons of my service improvement. I gained feedback from the patients family members as well as staff on the ward and on other wards. I feel that I have used a variety of sources to gain an accurate and reliable result. Study Merging all of my information/evidence together my main priority was to achieve a summary of the results. I used a matrix framework to bring themes together from the data I collected. This way I could set out the data in various categories to make the research basic. Furthermore with the information, I shared this verbally during multi-disciplinary meetings to put the service improvement plan across a variety of health professionals, gaining a professional feedback. Also discussing the service improvement with family relatives, gaining more of an outside view from individuals that don’t work in the health care. This type of study allowed me to gain precision and feedback from different sectors. Act Unfortunately as I am unable to actually perform this service improvement, therefore I need to look at this service improvement plan hypothetically. Reviewing changes of my service improvement plan I am fully aware that this service improvement plan will only work if the ward works collaboratively. Consequently the ward chosen for this, need to aim to improve their leadership skills and their collaboration between the other sectors. To help implement this plan effectively I am going to firstly introduce this plan into breakfast meal times, allowing me to improve small areas more effectively and then eventually open this plan out to all meals. Overall I believe that allowing 15 minutes one-to-one time, whether that is during all meal times or just breakfast will improve patients nutritional needs, especially for dementia patients it allows time for familiarity for the patients. Conclusion Concluding the whole assignment together prioritising the main issues in this assignment, I feel collaborative practice needs to be used as daily activities within the health care system. It has been clearly shown how essential it is to collaborate in a team and ensure leadership is prioritised. Designing a service improvement plan was a great experience and I now feel confident critiquing services and planning a change, it has helped me realise how much you actually notice during practice placement and the improvements that I, as an individual, can actually make. Overall, l I now hold a greater knowledge about team dynamics, areas of good and bad practice and service user involvement.
